To obtain a Certificate
of a Non-Criminal Record, the following must be submitted:
- Completed
application form
- Passport
- Xerox of passport
- 3 passport
photographs [2" x 2"]
- File/case number and the exact address of the organization
where the certificate will be sent to (Embassy, Consulate
or Immigration Authorities). The certificate cannot be given
to the applicant or his/her lawyer but only to the institute
or body that requested it.
- $4 cash or money order
If the request for the Certificate is submitted via mail, and not in person, all forms and photos must first be authenticated by a notary.
Processing time is at least 12 weeks.
