For urgent matters, please call (415) 844-7510 and leave a detailed message, including your full name, Israeli ID number, phone number, and the details of your request, including departure date (if applicable).
In the event of a lost or stolen passport, you must come to
the consulate in person. Only citizens living outside extended
San Francisco Bay Area may forward this request by mail.
Be advised, the issuance of a new passport after one has been
lost or stolen is at the discretion of the Consul and may demand
an approval from the Ministry of Interior in Israel.
Please provide the following:
1. Declaration of Lost or Stolen Passport from the local Police
station
2. Declaration
of Lost Passport Form - completed and signed
3. Request for Passport Renewal
Form - completed and signed
4. Valid Identification (US passport, driver's license, etc.)
5. 2 passport
photographs [2" x 2"]
6. $100.00 cash or money order
Further, Citizens living outside the extended San Francisco
Bay Area must sign all forms in front of a notary and provide
notarized copies of valid identification.
