Mutilated Passport

 

Due to heavy seasonal demand, we will require more time to process each request. Please note the following estimated processing times:
PASSPORT: 2 MONTHS -- VISA: 3 WEEKS -- NOTARIZATION: 1 WEEK

For urgent matters, please call (415) 844-7510 and leave a detailed message, including your full name, Israeli ID number, phone number, and the details of your request, including departure date (if applicable).


If you have a mutilated passport, you must come to the Consulate in person in order to obtain a new one. Only citizens living outside extended San Francisco Bay Area may forward this request by mail.

Be advised, the issuance of a new passport after one has been mutilated is at the discretion of the Consul and may demand an approval from the Ministry of Interior in Israel.

Please provide the following:

  1. Declaration of Destruction of Passport Form - completed and signed
  2. Request for Passport Renewal Form - completed and signed
  3. Valid Identification (US passport, driver's license, etc.)
  4. 2 passport photographs [2" x 2"]
  5. $100.00 cash or money order

Further, Citizens living outside the extended San Francisco Bay Area must sign all forms in front of a notary and provide notarized copies of valid identification.
