Birth and Death Certificates

 

Request for a Birth Certificate

An Israeli Citizen born in Israel may request a birth certificate for himself. In case of a minor a parent can request the certificate. The request is free of charge and can be done in person or by mail.
  1. The person requesting the certificate must provide an Israeli passport for identification and must complete and sign a Request for Birth Certificate Form.
  2. When the certificate arrives (approximately 2 months), it will be sent to the mailing address provided by the requestor.
  3. Return Mail payment

Request for Death Certificate

One can request a death certificate of a family member who died in Israel. The request is free of charge and can be done in person or by mail.
  1. The person requesting the certificate must provide an Israeli passport for identification and must complete and sign a Request for Death Certificate Form.
  2. When the certificate arrives (approximately 2 months), it will be sent to the mailing address provided by the requestor.
